You could use a multi-meter to check you are getting a voltage at the A4 & 5 pins but I cant remember what it should be. If you had put a pic up I would not need to ask how is your pro mini connected to the shield hard solder or pins and headers. What was the setup when the magic smoke came out?...

Re: Unable to make quadcopter lift off ground
There are two methods for trimming, using the Tx trim or the stick trim. Both trim in a similar manner but they affect two different things. Tx trim affects the motor output from your Tx when in gyro only (acro) mode, so that when you throttle up all the motors throttle up at the same rate (remember...

increase max throttle to 2000 and let your Tx go all the way up too. Did you set throttle trim to min on your Tx. I'm not sure if it makes any difference or if its just a throw back from the gasser types who need it to set tick over. Acc need to be set up before it will stabilize you copter, I think...

Check all joints esp' solder joints that they are all good. Is it the same motor every time?, if yes then move the motor to another position and see if fault still happens. You circuit as is should work ok, only thing I'm not sure of is the i2c level shifter but that is only because sparkfun do an llc (bob 08745) and its a lot less money. Another option you could do is use a 3.3v pro mini and do away with the i2c shifter everything would be at 3.3v.

Just get a 5V all in one IMU 9DOF or 10DOF with then you can ditch the LLC and have less connections. Plenty to choose from on ebay for a lot less than individual items. Look in the connecting elements on the home page and you will find a diagram on how to connect everything.
